NEW AND RETURNING STUDENT-ATHLETES
This is to advise you of specific health requirements that must be
met prior to your participation in athletics at Farmingdale
State.
All required paperwork for student-athletes is as follows…
New or Transfer athletes: College Health and Physical Report completed by your physician, an EKG with medical clearance to participate, and Medical History Form/Insurance.

Training Room)
NOTE: The Health and Wellness Center will provide EKGs for new and transfer athletes, free of charge, by appointment only, throughout the summer, once your completed Health and Physical Report is on file. Health and Wellness Center summer hours are Monday-Thursday 8am to 7:30pm, last appointment 6:30pm, and Fridays 8am to 4:30pm, last appointment 3:30pm.
Returning athletes: Athletic physical, and Medical History Form/Insurance. (No EKG).

NOTE: Athletic physicals will be available free of charge at the Health and Wellness Center on
- Tuesday, August 4, 2009 from 5pm to 8pm, by appointment
only.
- Wednesday, August 5, 2009 from 9am to 3pm, by appointment only.
