ATTENTION ALL NEW AND RETURNING STUDENT-ATHLETES

This is to advise you of specific health requirements that must be met PRIOR to your first practice at Farmingdale State.

All required paperwork for student-athletes is as follows…

New or Transfer athletes: 

1. College Health and Physical Report completed by your physician
2. Meningitis Form
3. An EKG, noted with written medical clearance by physician to participate
4. FSC Medical History Form/Insurance information

1. Health Physical Form - (completed by physician, return completed forms to Health & Wellness Center)

2. Meningitis Form

3. See below about info for scheduling an appointment for an EKG.

4. Medical History Form and Insurance Waiver - (return completed forms to Athletic Training Room)

 

Returning athletes: 

1. Athletic physical examination.
2. Medical History Form/Insurance.
3. No EKG (unless you had an abnormal EKG in the past).

1. Health Physical Form  - (completed by physician, return to Health & Wellness Center)

2. Meningitis Form - (return to Health & Wellness Center)

3. Medical History Form and Insurance Waiver - (return completed forms to Athletic Training Room)

 

 

Scheduling a Student-Athlete Physical Examination at the College

To make an appointment for an Athletic physical or EKG, please call the Health and Wellness Center at (631) 420-2009.  Available dates and times below.

• Every Wednesday from 10am-2pm, by appointment

• On Monday, July 19th, 2010 from 9am-3pm, by appointment

• On Tuesday, July 20th, 2010 from 3pm-8pm, by appointment

*You MUST call IN ADVANCE for an appointment.

*If you are having a complete entrance physical exam, you must bring copies of your immunization records with you.

*Please be advised that you can see your own physician for any of these requirements, instead of having them done at the College.
